Work Related Incident
- What to Do In Case of A Work Related Injury or Illness
- Get medical help, if you need it.
St. Alphonsus Occupational Health Network is the county’s designated provider for on-the-job injuries. A list and map of locations is provided on this web page.
Saint Alphonsus Occupational Medicine clinics are located throughout the region from Boise to Caldwell.
If Your Injury Is:
- An emergency: Have someone call 911 or take you to the nearest emergency room.
- Serious, but a non-emergency: Go to the nearest Occupational Medicine clinic
- Notify your supervisor of the injury or illness immediately.
Fill out an Ada County Incident Report Form:
This form MUST be faxed to Risk Management within 24 hours of the incident at 287-7129. If you have any questions, ask your supervisor.
What to tell the doctor:
You are an Ada County employee and Intermountain Claims, Inc. is the third party administrator of workers compensation claims.
- Return To Work Form—The physician should fill this out after each appointment with you. Ask the doctor to fax the completed form to Risk Management at 287-7129.
