FORMS
Family Law
Filing For Divorce - Forms needed to file for divorce with or without children. Form numbers CAO 1-1 through CAO 1-14 and CAO 6-3.
Proof of Service - Forms acknowledging or attesting to service of documents on the defendant. Form numbers CAO 2-1A through CAO 2-3.
Finalizing the Divorce - Forms need to finalize a divorce by default or stipulation. Forms CAO 6-2 through CAO 6-10, CAO 7-1 through CAO 7-5, CAO 8-1 & CAO 8-3, also instructions and default check list.
Enforcement - Forms used when enforcing custody orders. Forms CAO 5-1 through 5-8 and CAO 9-1.
Modification of a Decree or Order - Forms used when modifying support, custody and/or visitation. Forms CAO 10-1 & CAO 10-7.
Custody/Support/Paternity - Forms used to establish custody and/or support when parents are not married, to intervene in a case, join a party to a case or request genetic testing. Forms CAO 13-1 through 13-12.

Landlord/Tenant
Landlord Forms - (Expedited Eviction) - Eviction forms on this page are for evicting a tenant for non-payment of rent only (no other issues). If the tenant pays and rent is made current, you can not evict the tenant with this process. You cannot sue for back rent with this process. These forms cannot be used to remove a mobile home from a rented space. If you need to evict a person for any other reason or need to remove a mobile home from a rented space, you should consult a lawyer for legal advice. These forms are specific to Ada County. Check with your local court before using these forms in another county. Forms CAO 16-1 through 16-9.
Tenant Forms - (Claims Against a Landlord) - Forms used to respond to a complaint for eviction, demand return of security deposit, and complaint for specific performance (repairs). Forms CAO 16-6 and 16-9 through 16-19.
